Client Intake Forms (in PDF):

Please complete the appropriate forms and mail or drop-off at Dr. Janet’s office
3 days before your or your child’s first appointment.

The following forms take about an hour of time to complete – plan a quiet time with a cup of tea! Just choose the appropriate form.

If you have trouble downloading them, we have hard copies available at our office which can be mailed to you or picked up.  Be sure to include a list of your supplements and medications and current dosages.
